Course Changes

You must submit a separate Course Change Form for each type of course change (addition, deletion, revision); however, within that single submission, you can submit changes for up to 10 courses, as long as they are from the same department/program and are undergoing the same kind of change.  

  • Course Change Form Worksheet -- Use this document to guide the collection of all the information needed to complete the online Course Change Form. The department chair or program coordinator should provide the information on the worksheet that is relevant to the proposal and send it to the college/school Graduate Council representative.
  • Potential required attachments for the online Course Change Form (consult the worksheet to determine which attachments you will need to complete) -- These attachments should be completed by the department chair or program coordinator.
  • Course Change Form - Graduate Council -- the link to the online form. The form should only be completed and submitted by the Graduate Council representative from the college/school proposing the course change.
